I just reproduced this same 'disk read error ocurred' with disk2vhd 1.4 - it was on a dell poweredge SATA drive ntfs system partition.  What it turned out to be was a corrupt boot sector in the VHD.  This caused the vhd partition to appear as 'not formatted' with an unknown filesystem.
 
So it appears that somehow the boot sector on the destination vhd is getting corrupted during the conversion - seems intermittent and related to the source drive somehow. 
 
I was able to fix the vhd by repairing the boot sector using testdisk on ubcd4win.  It now boots fine in hyper-v.
 
Hopefully disk2vhd can be updated to check for common boot sector corruption during the conversion and fix it on the fly.  Thanks!

I havent tryed the HAL method above but with v1.3 I get a black screen. I attatched it as a second drive to another windows machine and it had the red circle with a question mark icon in the top corner of explorer when I browsed it in explorer. I am using VPC2007.

Received yesterday mine windows 7 ultimate DVD... and guest what I did first !!!!
Yep.... but NO.
Still no succes with Disk2VHD from mine IBM ( Lenovo ) T60 laptop....
Even not in WIndows 7
 
I am done...... can't use it Cry

Still no progress....
Only able to boot in Safe mode after doing the HAL....
 
Note:
With VMWARE I don't have any issues....
( But somehow i preferred this tool.. don't ask me why Embarrassed )

Eureka.....
 
- Created a VHD with the new Disk2VHD 1.3
- Try to boot.... Black screen only... ( So no error "A Disk Read Error Occurd" )
- Booted with a CD to repair XP with the recovery console
- Replaced the HAL
--At the command prompt, type “CD System32″ , hit enter. Then type “expand D:\i386\HALACPI.DL_” , hit enter. Finally, type “copy HALACPI.DLL HAL.DLL”, and hit enter. Type Y for yes on the overwrite file confirmation message. (*cd drive letter may vary)
 
Now it hangs on the Windows XP screen. Sleepy... but.... a little bit closer....Embarrassed
I am sure... this can be solved also

I wouldn't bother with the Virtual Disk Wizard anymore my problem was related to NT4 unable to handle large hard drives and nothing to do with the problems concerning other Operating Systems.
Disk2vhd always creates a 127GB hard disk image no matter what the original drive size is but the partition it creates is the same as the source partition.
By converting my VHD to fixed-size I had effectively changed the physical size of the drive to match my partition size thus making my hard disk image bootable for NT4.

I understand there will be a new version of disk2vhd soon which addresses some of the booting problems experienced by many here.
